Injectable methylcobalamin B12 is generally well tolerated. Rare effects may include headache, mild gastrointestinal upset, or tingling sensations, particularly if dosing differs from clinician guidance.

Rotate injection sites and follow technique instructions supplied by your prescriber and pharmacy.
Precautions
- Avoid use if pregnant, nursing, or planning pregnancy or breastfeeding unless specifically directed by your provider.
- Stop and seek urgent care for rash with breathing difficulty, lip or tongue swelling, or rapid heartbeat after injection.
- Discuss polycythemia, cobalt sensitivity, active malignancy, or concurrent folate replacement plans with your clinician.
Drug interactions
Fully disclose prescriptions, OTC drugs, nicotine, hormonal therapies, oncology agents, proton pump inhibitors or metformin when relevant for absorption labs, antimicrobials, and herbal products.
